Dear Parents/Guardians,
First let us thank you for the privilege of allowing us to care for your child during the school day. Responsibility for providing medical care and relief beyond first aid is strictly the obligation of the parent or legal guardian. It is VERY IMPORTANT that we have emergency information including phone numbers of family or other designated persons, parent’s work numbers, and the name, phone number of your physician and hospital of preference. In case of accident or sudden illness, the school nurse will give first aid and will contact the parents or guardian who will make arrangements for the child’s safe transportation home or to a medical facility, depending on the child’s need.
The District Policy on communicable diseases is governed by the Rensselaer County Health Department. The school acts on the recommendations of the Health Department when a communicable disease is reported.
